Do i need to declare uber/skip/doordash etc?

Hai filing my citizenship application have few questions if anyone could help me out!

1. I understand self employment must be declared. I used to work at these food delivery jobs part time and i still do once a while. Do i need to declare all this in separate boxes? Like Uber/skip/dd?
What should i put on the employer address column? If its my house address what if i moved few addresses around?

Yeah sure, list them. Whether you list each company separately or together probably doesn't matter.
You can put N/A for the address.
