Hi guys, I have not lived nor visited my home country for the past 10 years for more than 6 months in a row. Would I still need a PPC from my country?
 
Yes, you need it as your passport is from that country.
after getting marriage the legeal practice in Pakistan is to replace father name to husband name same in adress bar so it is also necessary for spouse to apply in both cities for PCC? As she is primay applicant

Yes definitely. Get PCC from both cities in Pakistan. Also attach a document enlisting a proof of name change.

which sort of documents do i need to prove this replacement? well it does not effect on my spouse name its a replacement in row not last name.
 
AFAIK, one PCC for one country,regardless of cities lived in.
About name, get PCC with name which is on spouse passport. Pre-marriege name is mentioned on Marriege certificate. You can mention in LoE but they know, you not only one.
Further different people different opinions. Seniors can shed a light.
