Do we have to crop down the image to the specific size or just submit the full scanned page as is?

Right now I have giant blank page scanned at 600dpi with my picture in the corner. Is that alright?

has to strictly as mentioned in document.
 
You need to fix your image with one of the online image resizing tools or just download GIMP and do it yourself.
 
Are you sure? It says "at least", so that strongly implies bigger is okay.

(I mean, it's clear that the image should just be the image, not a blank page with the image in the corner... but the actual 100% of the picture doesn't need to be only 35x45cm, I think.)

Take the photo home, don't waste your money.

As per CIC:
At this time, the electronic photos uploaded by applicants cannot be used for the permanent residence card; therefore, CIC will not reject an application as incomplete if the photos submitted do not comply with CIC's prescribed photo specifications.

we would be sending physical photos during PPR, so electronic copies of the photos do not need to strictly adhere to the specs

go to some studio shop any pharmacy store or walmart will be okay...

and yes minimum size is 35 by 45 so if your photo is according to specification but during scan size is bigger thats ok just crop it but no need to be precise you can just verify your photo in any online photo editor tool for canada specification to check neck and ear are in that frame size rest is okay.... :) :)

at the end they will ask you for printed photos during ppr so do not worry.
