Me and my new sponsored wife had some issues that were finally resolved but in the interim she had left me because of depression, and a medical condition and wanted to be with her mother for the surgery back in her native land.

During the course of our fight, I had called immigration because at the time I felt things were not progressing along. Immigration did ask me to send a statement which I did not do. They provided me a reference number to attach to the statement.

What happens now because my wife wants to return. Will she be refused entry to Canada?

It depends where you are in the process. If she already has a PR card I would think you would be okay as you probably only spoke with a call agent. It sounds like she is from a visa free country or has a visa so returning should not be a problem. If you are at an earlier stage you may get asked to clarify what is happening.
 
She was already sponsored and in Canada with me but did not stay for the 2 year conditional permanent residence. She left for the surgery and we had some disagreements that have now been resolved. She is scared to come back because I had phoned immigration and told them she left.

Nothing to lose by trying to come back. My "guess" is that you only spoke with a call agent and this was not recorded. That is just my guess.
 
So how many days did she accumulate in Canada, what is the status of her PR card, does she have travel visa or is she from a visa exempt country ? Answers might get more feedback

second that.. i mean this isn't something official anyway. at certain point like his; i am guessing nothing happens; if she comes back. although if she did not live enough days out of 5 years; then she couldnt renew her PR card anyway.
 
730 days if I'm not mistaken.
