When we fill this out on the airplane, is it one per person, or can we put everyone on one form? There will be two Canadian citizens (one infant and one adult) + my wife who has the PR visa.

For purpose of trip, do we check 'personal' or 'business'??

Your question is not clear.

Are they all of the same family? What is the relationship between the two (adult +infant) and your wife?
 
1) All on one form.

2) Personal.
 
Correct. The rule is 'one per household'.

Im my wife's sponsor (Im a Canadian citizen) and she has her PR visa. Our son is a Canadian citizen.

I understand the one per household, but does that apply if our last names are not the same? My son and I have the same last name, but my wife is still using her Chinese name.

I don't remember ever filling out one of these forms, but I guess I must have...although it was when I was alone and not with other people.

Yes, absolutely! Quebec is chock full of families with different surnames (they don't recognize marriage as a reason to change surname there).
 
Okay...thanks for clearing that up.

What about at the bottom where we need to sign? Since our son is only 15 months old, he can't sign...so do I need to sign for him, or just leave it blank?
