THe procedure is changing, so it's a bit all over the places! before the new procedure ( 1st of November), medicals were sent to a regional center, and then to the VO. They would "sit there" until the file would come from Mississauga... and the line "medicals have been received" would appear only when the sponsorship approval was done, and the file sent to the VO - so then it would be a "good way" to know that the file had arrived at the VO, and the medicals were matched.
now - from NOvember, the doctors send the medicals through an electronic system that makes it easier to match with the file. SO for the more recent medicals and applicants, they can see " medicals have been received" during the first stage....
In your case, it was done with the old system, so I suppose that they were sent to Pretoria and they are waiting for your file. Since you got the sponsorship approval, your file should be on its way to Pretoria and matched soon. However ( it's CIC after all!), some applicants never got the "medicals have been received" line on ecas, and their file was still complete... so if you see it on ecas, it's a good sign, but if you don't see it, it's not necessarily a bad sign!
