Custodianship minor declaration form Canada student visa

I am applying for Canada student visa,but I'm a minor. I have filled the custodian declaration form, but my father is deceased, what will I fill in the parent2/guardian2 column. Also what are the others documents that I need to attach with it. Please reply asap.

Parent/guardian is the adult who is responsible for your care in your home country. Custodian is the person who will be responsible for you in Canada, read the links for requirements. You might need to explain your family information and include proof in your explanation letter/study plan.

Please give instructions on how to fill the form IMM 5646, when parent 2 (father) is deceased.
If you only have one parent and no one else who is responsible for you put one name.
Explain in your letter and include proof i.e death certificate.

I'm also in dilemma regarding the info to be filled in custodian declaration form. Do We have to fill the information according to the passport.
The custodian i'm getting is a PR and is having an indian passport. Do he have to fill the address according to the passport or his canadian address?
