Write whatever you are doing nowadays. House wife/House Husband, Business person, Studying or whatever you are currently doing.
Your NOC will be calculated in the Past 10 years. So hopefully you were functional as a doc in the past 10 years for at least 1 year (according to my understanding).
When applying you have to prove, that you were working under required NOC, your duties fall in that category, AND you were paid. You have to provide Pay slips, salary letter or something that assures that you had a paid job.
