Covering letter and Index sample for OINP application

Can some one please help me to get a sample covering letter and an index format
I have got PT now need to submit my docs so I need these.

If you have 453 or more CRS points, then kindly ignore this PT NOI as you will get the ITA through the rounds of invitation soon in the next few draws.

3. Language Requirements
4. Settlement Funds
5. Intention to Reside in Ontario

Kindly apply for OINP. Make sure your application reaches within 45 days of receiving PT notification of interest.
Till date, none of the draws have seen the cut-off score below 450 points.
