1. For applicants currently in Canada (a copy of your Work Permit, Study Permit, Temporary Resident Visa, and/or any other Canadian
immigration document or entry stamp must be provided with your application package) and below this
Please provide the Unique Client Identifier number (UCI) issued by Citizenship andImmigration Canada, also known as a client ID number.
I had visited Canada a year ago and had a visitor visa which has expired should I be mentioning that UCI number here ?

2. Birth Certificate
My BC does not mention my mothers or my surname only first name , but has my fathers name along with surname, would this do ?

3. Proof of family ties
I have a sister in Canada but dont have her BC, but her passport mentions parents name. Would this be ok?

4. My address does not fit in the space given in the form and it doesnt accept N/A in some places, can I manually add it later after printing?

5.Certified true copies of applicant's university degree(s), college diploma(s) and/or occupational certificates.
Do I need to give notarised copies of my bachelors degree along with ECA ?

But the question says for applicants currently in Canada. I'm not in Canada and my visitor visa has expired too.

Please write 'N/A' and provide your UCI No. to the next question.
 
For additional info, they have asked to use attachment ? is there a format for this or do we we just a page with the details ?

Same format as the form.
