I am from Iraq and one of my passports was issued to me in Baghdad, Iraq and the second was in Ottawa Canada.

What should be my County of Issue & Place of Issue for the 2 passports ?
 
Refer to the application instruction guide. It answers this exact question. Why does no one want to help themselves! If you follow the guide you know you'll be doing the right thing!
 
Here's the guideline...

14. Tell us if you have held passports or travel documents during your “eligibility period”.

  1. Check either yes or no.
    • If you check yes, complete the chart labeled Table A in question 14 by including
      1. the document number
      2. the name of the country that issued the document (issuing authority)
      3. the place (city or town) the document was issued
      4. the date the document was issued
      5. the expiry date of the document
Thus:

Passport 1
Country of Issue: Iraq
Place of Issue: Baghdad

Passport 2
Country of Issue: Iraq
Place of Issue: Ottawa.

The problem is passports these days aren't issued in a particular location. Mine were both issued by mail and don't say a place of issue.
