Consent Form for GCMS notes

Hey all,

I applied for the GCMS notes and received this email saying they cannot process further.

"The consent of ******* is required to process your request. To be valid, a consent must be signed, dated, and indicate to whom authority is given with the printed name of the person giving consent."

"Your file will remain open for 30 days from the date of this letter. If you do not respond within this timeframe, we will consider your request abandoned and will close your file."

I put ******* cause that's where my wife's name was located. Is there an example someone can give me on how this letter should look like and what is needed to be written on it??

Any help would be appreciated.

If the sponsor requests for GCMS, does he/she still needs to get consent from the principal applicant?

Yes, the GCMS is for PA, thus the consent - it can only be ordered by Citizen or PR

Here is the link to the authorization form you need to complete and send http://www.cic.gc.ca/english/department/atip/form-imm5744.asp. A new one needs to be completed every time the GCMS notes are requested.

Yes, give consent to this sponsor to order it on their behalf

I applied the ATIP notes as the applicant i cant order that & not allowed. but i used my husband's credit card informations & made it online. you have to submit all of sponsor's informations & order it online. you choose the electronic notes, then it'll come over the email & takes about 1 month only.
