Consent for gcms notes?

Hi i am a canadian citizen living in canada. i sponsored my wife from pakistan in Nov, 2012. i wanted to know if i order gcms notes for our case, would i need to get my wife's consent for releasing private information (IMM5475E form) before making a request? not sure how to order these notes..

Have your wife fully complete the consent form, sign with date, and email it to you.

Does the applicant have to sign and put in a different date every time? Or can she just sign/date once, and then you just use that same copy with the old date, over and over again?

since you will be scanning the document each time, i don't see why you can't have 1 master copy with her signature, and fill it out each time you request the notes...

I want to know that if sponser person want his /her GCMS note self so she or he do not need consent form right? Also he or she do not need additional attachment of proof of Citizen ship?

Not if sponsor needs for their own file ( but that wouldn't show anything on the PR side, i.e, wife sponsoring husband ) as it will only show the details of their sponsorship approval

Yes, new date everytime ( telling you by experience as they returned mine ) - what I did was, made a digital signature ( jpeg of signature made via signature saver app on android ) and inserted that and date in Nitro PDF - so no more hassle of asking consent
I ordered 14 GCMS notes before approval

yes, thats the whole point about consent form
