Okay ill choose legally separated. It also says on the form, if i am married I should include my marriage certificate or contract.

Should i include my marriage contract on my application form?

I believe you refer to your current relationship for this. I am not familiar with how the forms are specifically filled out for conjugal. You may want to look at some other conjugal threads.
