Conjugal Sponsorship : Maiden versus Married

First of many questions. Principle applicants Philippines annulment denied for *THIRD* time. After 6 years of heartache we are turning to conjugal sponsorship. Without going into too many details :

- Principal Applicants maiden name is on her birth certificate (obviously)
- Marriage Certificate shows Married name
- Sons (16 years old) birth certificate shows Married name
- Passport (due to expire soon) shows maiden name. Can obtain new one with whichever name makes more sense
- Failed annulment paperworks show maiden name
- backup documention to prove our relationship (years of remittance slips, cruise ship documents, etc etc ) all have maiden name.
- Unsure what current Cenomar name shows. Unsure if I even need to provide.

So basically only her Marriage Certificate and Sons birth certificate show married name. Father will be signing notarized IMM5604.

Under which name (Married or Birth) should all IMM documents reflect? I obtaining new passport (it expires early 2019 anyways) name should match IMM documents?

She should apply using whatever name she normally uses. There is a field for "other names known by". You can also include a sheet of paper explaining the different names on documents.
