1. When they say in an application "You and your family members", are they asking strictly about spouse and children or are they also asking about brothers and sisters and parents?

2. In the country specific application (China), where did appendix C & D go? There used to be a form for the Police report and a form for Medical check. Now they're gone. What does that mean and what do I do?

3. Also in the country specific application (China), Question 9, Citizens of the People's Republic of China must provide photocopies of all pages of the household register ("hukou")... they want this translated in English?? That's a lot of pages and a lot of money for translating. Or am I mistaken and just want the photocopies as is?

4. In IMM5669 Schedule A background declaration, in question 8, what do you say the name of the company if the wife was unemployed and stayed home?

5. IMM5490, Question 27, Hotels and Parents' house count?

I really appreciate your help to whoever has answers. Thanks in advance
 
Family member: spouse and children. Your siblings and parents are only listed in the additional family information form.

I'm not familiar with Appendix C or D, but medical checks should be done upfront. The same for police reports. There used to be a form you needed for medical checks, but that procedure was changed (last year I think?) and now you don't need to bring any forms to the doctor.

All documents not in English must be translated and notarized. If your household register is not in English, then yes, you need it translated.

For IMM5669, my spouse put unemployed in the title box, wrote the city, and left the rest blank.

I'm not sure what you mean by hotels and parents' house count.

When you said "must be translated and notarized" What needs to be notarized exactly? The translation?

For IMM5490 question 27: it asks if my wife and I lived together. We did so a month at a time when I was visiting China. The first time we went to hotels mostly and then the parent's house. Do those places count as living together?

Also another minor question. When they ask you to "attach" an extra sheet of paper for any additional space required, do they mean I can just make a random paper as long as it says the name of the sponsored, title of the form and number of the question? Also, do I staple the extra sheet and all the applications or do I use paper clips? I don't know how picky they are.

I'm not certain how Chinese notaries work, but in Russia, they notarized a copy of the original, and then it was translated. The translator had to translate the notarization.

If you were together for a month, I would put it as living together.

I made a Word document, wrote the principal applicant's name, DOB, the form number and question number, then wrote whatever I had to write.
I've read on here that staples are a no-no. I used paper clips.
 
lol MilesAway.... I'm not certain how it's done in China either. However I did go over it with her and she is deciding to do the same. Get it notarized and then translated, even though they are not asking for a notarization. Just to play it safe.
Thanks for the staples tip. I will use paper clips. However, I did use staples on my chat logs and other proof printouts. But I think I will remove the staples just to be on the safe side and just use paper clips.

-In the "additional family members", They wanted names and addresses in both English and Chinese but they don't fit. Instead of doing a word document, I used another form just like it except I filled up the information in Chinese and will attach it with the English version. Is that ok to do it that way? I also left my information blank in the Chinese version since all my information can only be in English.

In the form IMM5540 Sponsor Questionnaire, Question #8 "Have you resided in countries other than Canada, if yes, give details.....
I assumed I'm supposed to just simply type what they asked for. Yes I have lived in another country off and on. But why did the immigration consultant recommended I also add all my addresses in Canada? She pretty much made me start from my birth up till today writing all the addresses both outside and inside Canada. She said she is certain they will send back the application for not being filled properly if I don't add my Canada addresses.
Does this seem right? I mean I did it anyways, but I'm just curious why I was recommended on something they didn't ask for.
 
We didn't even use paper clips, they'll really weigh down your application. Simply put your supplementary pages immediately following the form they belong to.
