confused regarding the various forms to be filled in and checklist

use the first one if ur applying under SPP. second is a general checklist and not specific to indians. if ur from india and applying under SPP use the first one

the second one asks
"Any additional documents required by the responsible visa office" so it is directing u to ur visa office.

u need to follow first checklist or else u wont use GIC , IELTS and other required documents and ur application wont be accepted at vfs center.

Student Questionnaire Form isn't available per say for download but the format is available on this link, http://www.canadainternational.gc.ca/india-inde/visas/questionnaire-study-permit-permis-d_etudiant.aspx needs to be copied, pasted in word ,filled and submitted in your application package..
