Access to info and personal info request?

Hello all,

May be is a stupid question but I`m getting really confused with all this papers...Does any one know how to fill section C on page 2 of the following: http://www.cic.gc.ca/english/pdf/kits/forms/IMM5563E.pdf
My husband is sponsoring me and he is filling up this documents. So does he need to put my name and details here or still his?

Hey Blagogalia,

I just did this yesterday. The information requested is your information (because you are the applicant). And what you are requesting are the electronic files if you want the GCMS notes.

You must be a Citizen, Permanent Resident or a person currently present in Canada to request the GCMS notes. If you do not live in Canada then your husband can request it on your behalf. You will need to fill in consent Form IMM5744 authorizing your husband to obtain the information. Your husband can then submit IMM5563 and IMM5744. There is $5.00 fees for the request.

If your husband Is making the request then he will fill in his information in section B and your information in section C.

If you are in Canada then it will be your information in both sections
