The guide states:
A permanent resident visa is issued for a period not exceeding the earlier of
•the expiry date of the medical results for you and your family members or
•the expiry date of your or your family members' passport.
Permanent resident visas cannot be extended once issued. If applicants do not use the visas within their validity period, they must re-apply for immigration to Canada.Their sponsor will have to submit a new sponsorship application and pay new processing fees.
But the site then states:
You can only use your examination results in your application for 12 months from when you had the examination. If you are not admitted to Canada as a permanent resident within this time, you will be required to undergo another examination.
Does that mean that the permanent resident visa is only valid as long as the medical is valid?
 
That's correct. The visa will expire at the earliest of these: a year after the medical exam, a year after the chest X-ray, or your passport.
 
So... how does he stay a permanent resident for the three years until he can apply for citizenship?
 
The expiry date on the visa just means that you have until that date to land in Canada.

Once you land, you become a permanent resident (PR) and will be issued a PR card. Your PR card is valid for 5 years and can be renewed.
 
okay thank you... that's what I was wondering. The wording is confusing where it states:
