I will be sending in an application for my spouse soon, so I am wondering if I should hold off on getting his medical check until they ask for it, since it will be a LONG time (over 20 months) for them to review his portion (and it will expire by then). I don't want him to get it done twice (since it is far away to get it done and I don't want to have to pay for it twice). Any advice??

where are you getting the 20 month timeline? are you applying inland or outland? if outland, where is the applicant from?

I'm applying Outland as we are in Egypt.

The medical is on the checklist as a required item. Not including it means you risk having the application sent back because it's incomplete. Get the medical done ahead of time as per the instructions.

Yes, you should do them upfront. A lot of the time, if the medical expires before they are finished, they will extend them. Also, the CIC time estimates are usually way longer than most applications actually take. For instance, Ottawa currently says 15 months, I believe, but looking here in this forum, most Ottawa applicants get theirs done in 6-8 months. Best to go ahead and do them. If by off chance it takes over a year and they don't extend it, worst case is that they ask you to do it again.
