co op work permit problem

i got my co op work permit but there are some errors on it. like they did not mention my surname on it and its expiration date should be until august 2014 because my study will finish at that time but they mentioned expiration date of co op work permit december 2013 (my passport is expiring in december). plz help me how can i solve this issue

There are no date errors on your work permit. The validity of your work permit cannot be longer than the validity of your passport. Once you renew your passport, you will have to apply to extend your work permit.

1. Your Co-op permit duration can't be more than 50 percent of your course duration.
