clarification in filling IMM5562

hi,
filling Form IMM5562,
I am currently working in saudi arabia for the last 7 years residing with family and took vacation every year, while filling this form

how to fill this form 1. single filling - From start date to current date
or 2.i have to skip vaction dates and break into seven times
3. do we have to include transit countries also

how did you fill this form then??
we also live in saudi arabia and travel every year for vacation to india
