Okay I filling out the new consent form for GCMS notes. Lets break this down.


The form is meant to be for the spouse being sponsored.
For example: I am the Canadian sponsoring my spouse so technically my spouse is suppose to fill out the form.

So section A is for the spouse being sponsored.

Section B is for the Canadian sponsoring the spouse.

Section C is the Canadian's information.

This is where it gets confusing, #4 in Section C says "Signature of Representative"

If I use common sense, we wouldnt sign #4 Section C because we dont have a representative. BUT Ive seen some posts on here saying that the Canadian sponsoring the spouse is technically the representative so they should sign anyways. I want to send in these notes and not have them sent it back. And im sure this post will help many others since something that is so simple is so complicated.

I am the Canadian sponsoring my spouse. I took the approach that ATIP is a new request/application. Therefore, because I was making the request, I filled out Section A, and my wife filled out Section B. We left the rest blank. I'll know by Sept 22 if this was correct or not!
 
They have requested a new consent form, with an explanation of how to fill it out:
Section A: Principal Applicant
Section C: Sponsor or representative.

I should note that my request was for both the sponsor application, and the PR application under the Privacy Act. That was how they told me to fill it out.

aye that is correct: A for the applicant (your husband), and C for the sponsor (you). the rest is left blank. i actually prefer this newer version of the consent form as it's a lot easier to understand compared to the previous one.

btw i'm going off topic here, but how are things going with your husband's application? i recall he got a remedical request in july. wouldn't that mean he's close to being finalized?

I don't mind them changing it but I just got use to and understood the old one! So I guess I have to sign #4. section c representative thingy? Cause there's no where else I can sign.


And yes we did re-medical. Our MYcic was updated on August 1st to say that medical has passed. Our background check still says in progress so I guess we are just waiting for security. Eagle mentioned waiting 4-6 weeks so we're waiting patiently ((kinda))

correct, your signature goes in box #4 (signature of representative). wives are the bosses of their husbands, therefore they must also represent them ;).

so i guess they're going through background checks right now. have they passed eligibility yet or are they doing both background and eligibility concurrently?

Hehe yes that is very true
We were done eligibility in December, so I'm not sure. I do know that they didn't start our security until later because my mp verified that.

ah, i see. they seem to only work on both assessments concurrently if eligibility ends up taking longer than expected.

Fill out Section A by person being sponsored (principal applicant)
Fill out section C by sponsor and SIGN ''REPRESENTATIVE'' or your application will be held until you provide an additional consent form that is signed. we don't have a representative but they requested this to be signed
