Question no:12b I marked yes at taxes filed for 5 years.Should I mention that required to file I need marked "No" or leave blank? Please help.

Did you file you taxes for the year 2017 already?

If you filed your taxes for 5 years, you should check yes to all "taxes filed" boxes.
As for the "required to file" boxes,
You should check all of them.
