CIC requests IMM5475 form --Urgent!!

Hi All,

I recently received an email saying that I need to upload IMM5475 form to continue my application. I did use representative through my application.
Does anyone know why they are requesting this ?
Also, for name of office where application was submitted. Where I should put down ? I uploaded all documents online though.

Any help is much appreciated!

Hello! Sorry for the late reply. What I meant is that I wrote “ONLINE” on the form, where they ask which visa office is processing the application.
It will be fine! Good luck!
