Hi,

I submitted my PR application on 16th Jan and received the AOR on the same day. Today I have got a message that CIC requires a background declaration Form - IMM 5669 in order to continue the application. Has anybody else received the same request? Was i supposed to upload this while uploading the documents? Pls help.

many people received the request to submit schedule A

Most of the ppl are asked to submit this schedule A form.. mostly it's asked when u have selected yes for few statutory questions.

I have checked through some timeline and realized is not everyone asked for schedule A form or maybe people dont include on timeline. Is this true?

Not every is asked for Schedule A, I wasn't asked either. Even if many people don't include on their timeline it is irrelevant to your application. :)

They asked me this form now!
I received my aor on 12th Jan.
Why would they ask for this form???

It is mostly asked from Provincial Nominated applicants, for background information.

hi, when you uploaded the schedule A for your spouse, it it appear under your name in the document list?
I am getting a little confused with this and your help would be appreciated :)

Yes it have to be uploaded under your name. and in some caes for independents too.
It was my confusion too, but since I found out that all requests from CIC have to be uploaded under the principal applicant, I did so and it was all aproved.
Seems to be the way how CIC it wants to have.

Just to echo most of what's already been said -
*Not everyone gets Schedule A request;
*Those who answered "yes" to one or more statutory questions (eAPR) seem to get Sch A request;
*Upload via principal applicant's section of the documents list (this applies for all other additional documents request too).

Best wishes to all.

Hi Suarez,
I believe this document will be asked not only if you answered yes to any statutory question, but I think if you fill any gaps while giving last 10 years history. Same thing happened with me, I did not put yes to any statutory question. I filled with some months gap.

I got a question here, if my actual eapr and the schedule A doc has any date differences will they find its a misrepresentation? Because I felt there shouldn't be any gap and filled the doc to avoid gaps by just clubbing days.

Thanks in advance
