Chronic kidney Disease-stage4.how to respond fairness letter?

Hi Guys
Please help me.I just receieved a fairness letter.I applied for my parents and my Mom is diabetic and has blood pressure.During the exam they found high creatanine level and referrred for nephrologist.

Nephrologist told that kidneys are Normal despite the high creatanine.Now the letter I got says My Mom has stage 4 CKD.I have 60 days to respond to this diagnosis and excessive demand.

Please give me suggestion what steps I should take now and any links to how to fight this.
