During the process of renewing the Child Travel Document, the passport office called and required two forms (PPTC 028 & PPTC 0473) to be signed by a notary and sent.
Both forms seem to not exist online anymore? Has anyone been through similar scenario and have these documents been replaced with other ones from your knowledge?
