We are planning to land in Halifax in mid February 2019. I tried to down the child immunization form but to not sure that this can be used in Canada to fill up details by Doctor in India .

You should get the immunization record from your Doctor. Every pediatrician has a record of what vaccines are mandatory in your home country and can provide you a report of what have been completed and what are pending.

and not sure why Doctor requested us to get the form for Canada to fill up the details ..
 
I feel the doctor is only trying to be sure he gets you the right content that is acceptable by the Canadian Authorities.

Please tell him to ensure the following points and you should be good to go:
Immunisation Record preferably should be typed or handwritten but capitalised

The record should be on his her letterhead with his sign and stamp (having the mandatory medical council registration number)

The record should have the proper name of the child s per the passport with age as on date

Though we don’t need to mention as in India we follow the WHO EIP guidelines which are universally followed but he she should ensure immunisation is up-to-date

Please don’t worry about any non-mandatory vaccines if skipped you will not be penalised
