In the university checklist it says complete and place this checklist on top of your application. does that mean I have to put all the 8 pages? theres useless stuff like how to get gic and minor signature etc.

i think just first page...the one having the contents as..
applcation form....medical...n all dat...
not all other pages

The first four pages are relevent since you have to put a tick next to those. Can somone confirm what to do here?

i don't have any experience regardng this ....but i think pages on which box for check is there ..all those pages has to be added
