It applies to everyone if u read the form again its written that you have to mention your name in Native Language also.

If the sponsor is a Canadian Citizen and child is born outside Canada, then child is a citizen. In this case you will need to apply for child,s citizenship certificate.

If the sponsor is PR of Canada then should add the child in PR application of the wife.
