Explain them the situation and keep patience. I know it is very hard to remain patient in this situation but I pray you get your medical call or visa in due time. It seems highly improbable that you will reach for your classes in time. So better ask your university if it is ok to attend classes a few weeks (2-3) late. Explain them the situation and if the agree try to relax a little and keep calm, but if it still doesn't shows up despite getting some relaxation from university, you have no other option but to defer your admission to the next term.
