Change of Name Query

Also if I don't then where do I upload the Affidavit ?

Ok...The way i see it is this.


Birth certificate name: Rakesh Khanna
Passport name: Rakesh Khanna Krishnamurthy
Name on degree: Rakesh Khanna.K
Letter from bosses: Rakesh Khanna

This is excatly my scenario. I am in two minds if I have to give an affidavit of "one and the same person" or just an LOE.
