My wife is on the family application as a co sponsor but her last name on her passport is still her maiden name. When I try to verify the form is says name doesn't match, therefore it won't verify the form. What should I do?
Can I send in the application unverified and on a separate form explain?
Is there other supporting documents I can send so it doesn't get rejected?

Do it verify if u change the surname of ur wife superficially to same as yours in the form? Just to croess check if that is the issue
 
Maybe put her name on the form as her maiden name, list your name as 'other last names used' and include a note?
 
thanks but it still doesn't validate it. If I send in a non validated form with a note will they reject it? Is there a special form where you add notes or can I just use typed documents?

That doesn't make any sense. If you only have her name entered as her maiden then it has to match. Your wife's name doesn't have to be the same as yours, so something else is awry here. Do you have her last name as not her maiden name anywhere?
 
thanks looking it again I changed one was caplocked and the other was not. Now it matches however does my wife sign her maiden name or married name? And do I have to change her name for other forms like IMM 5768. Tax notice is in her married name? Thanks!
 
I'd say sign her legal signature. Otherwise.. I don't know man. I'm sorry. This seems logical to me but I don't know what they'll be like and I'm afraid of answering wrong. I'd leave her legal name on other forms and just, as I said, add a note after the form you had to switch to maiden.
