Hello. I'm a Canadian currently living outside of Canada. I would like to order my husband's CAIPS notes, can I do that or must I have someone in Canada order them for us? Also in what form can we receive these notes? Can we get them via email or are they hard copy?

You can order for CAIPS (now called GCMS) notes on behalf of your husband. Along with the Request form and Use of Representative form, you would also have to include a copy of your driver's license or other proof that you are a Canadian Citizen. You can get the GCMS notes in email, if you wish.
