Can I order GCMS notes for my wife?

Hello, I am the sponsor, just a quick question can I order ATIP notes for my wife, do i need to fill out the consent form. What do I need to do?. any help is appreciated. Thanks in advance!

I have always ordered the notes for my husband. Just make sure as others have said is to get the consent form signed, unless you signed to be her representative .

she lives in Pakistan, so how will this work. She prints the form signs it. Send it to me via courier and then I scan it and upload it is that how it works

It will be faster for her to sign, print, scan and set it to you and then you can attach it to the request which you will submit

yes, email/scan copy is fine, i have always used that.

I don't have a scanner so my husband prints it out signs it then email to me and i take a photo of it then attach that to the request . It has always worked for me .

Some time ago, I found this info regarding notes on this forum, so I copied it and saved it for future reference. I didn't write it and have not tested it, so I'm not a 100% sure if it's completely accurate or not. Here you go:

How to apply GCMS / ATIP notes.

Only a person who is in Canada can request the notes.

If principal applicant is in Canada, he/she can request for himself and apply for free under Privacy Act.
If sponsor is requesting it on behalf of Principal applicant, you need consent form from PA and $5 fee. You can receive consent form from spouse thru email and upload it.
Apply under Access to Information Act.

It takes about 30 days to receive by email. It is a ~30 pages document

Consent instructions:

Section A: Applicant's info (sponsored person right)

Section B: Does not apply for spousal sponsorship. (it says if you need your spouse information then fill this)

Section C: sponsor's info (the person who is sponsoring)

Section D: Email (there is no email on section D)

Section E: not applicable
Section F: Applicant needs to sign

there are no section e and f

if we use electronic signature even though my wife signature is different its still ok or no

please help on section A,B,C,D
