Can’t get IMM5604e

Hello. I’m trying to apply for PR via PNPNL program. I have a child but she lives with her father and she won’t come to Canada. When I applied for visa I wrote there that I have a child. But now I can’t provide IMM5604e
And child won’t come to Canada. The system requires this paper. What should I do? I only have her birth certificate .

You need to locate them together with your child and the father. The father should sign for this form that he is aware that your child was no longer want to come with you, this form will need to be witness in the front of attorney or any law firm officer.
