anyone here from pakistan? i have the B (Bay) form in urud and right beside it has eng written as well. im guessing i can use that as my birth certificate?

I believe that should not be a Prob . You can use it as your Birth certificate
 
I think as long as corresponding to Urdu - adjacently English Translation is provided at the time of Birth Certificate Issuance its not a problem .
 
But it has all my siblings and parents names and date of births and also my parents id numbers. My parents permanent address of back home when they lived etc. so would i need a separate certificate or i guess this B form would be fine?

Yes, I am from Pakistan. I submit copies of my Birth Certificate in Urdu, B Form, Birth certificate true translation in english, Nadra issued computerized birth certificate (all notarized) and it worked for me.
