Birth Certificate Name Issue

Hello Everyone

I applied for Quebec Skilled worker and I was asked to provide birth certificate. The problem is my Birth certificate only mentions My first Name, Parents First name.
I reside in Canada.

Will I need an additional affidavit that says about my first name and last name missing from the birth certificate (and same for my parents?)

Can you provide me the format? Can i get this notarized from India?
