You will need to file an affidavit and supporting documents in support of the affidavit.

An affidavit stating the common parents, and supporting documents such as passports, graduation certificates etc. to support the affidavit. An affidavit alone will not be sufficient.

Thanks for your reply.
Do I need to create two separate affidavits of parents? Also passport would definitely have different name so will it be acceptable as supporting document?

You can get two affidavits, one each from your parents.

You can also get an affidavit from your sibling. All you need to show is that there is a spelling error in the birth certificate, and show that the exact spelling is different, from documents such as your parents birth certificate, your siblings high school certificate, or secondary school certificate, sibling's passport etc.

What if I only upload passports of me and my brother?
Our parents name are available there and exactly same but I don’t have middle names of parents in my passport

My passport- first name last name
Bros passport - first name middle name last name

Will it be ok to upload only passport in this case?

Yes, that would be better, and you can get an affidavit from your parents affirming that they are your and your sibling's biological parents and known as first name, last name; and first name, middle name, and last name.

Thanks. I think that would solve the issue. But is affidavit necessary as already names are same and just middle name is missing. Can I just use loe to explain that or do I create affidavit as process would be longer in this case

You can use LoE, however, the affidavit is legally binding document. It is entirely your choice, but I would recommend affidavit too.

Hello Pls. give us an idea. Canadian embassy needs to provide the correction of the name on the birth cert of my friend's application. But when she checked on the municipality/PSA it take 3-6 months. So the municipality said that they will provide a certification stating that the correction of her name on the birth certificate is now ongoing and she will also provide an affidavit for supporting docs. the question.. is it ok to send the cert. from the municipality and affidavit for the correction of her name?. Thank u. God Bless.
