Question 1. Ask for full name "Should i put my maiden name or should i put the name on my passport, using my husbands surname."

From the guide, the instructions for Schedule A--Background/Declaration (IMM 5669) for this question:

Indicate your full family name (surname or last name) as it appears on your passport, travel or identity document.

What I did, because my passport doesn't have my middle name, was to put my name exactly how it is in my passport and then I think the next section says "have you ever used any other name, alias, etc" so I checked yes and then wrote my full name in that section.

Thanks! i would be using the name on my passport then.
Just to clear things up these 2 forms IMM5669E and IM0008 both ask for Full name, both answer should be the name that appeared on the passport?
