Are Schedule A requests general or personalized?

The Schedule A request I got for my partner said this: Schedule A: A separate and newly completed Schedule A Background
Declaration form (IMM 5669) bearing an original signature. Please provide answers to Statutory Questions.

Is that the same as the one everyone gets? His answers to the statutory questions were clearly visible on the original application. Also, of course we filled in the entire thing, and not just the statutory questions. Was that what we were supposed to do?

Have people got different Schedule A requests directing them to different parts of the application?

I got a Sch A request and they pointed me straight to the statuutory question part. So I guess they can pinpoint where they need clarification. I forgot to mention some visa refusal

I just got a Schedule A request aswell for both me and my wife asking to complete question 6.

We had a visa refusal once for which we had answered yes.

My question is do I need to fill the complete Schedule A form or just question 6.

Here is from my request letter and I only submit the first page without signature as requested...so I think this letter should be personalized, please correct me if I am wrong. Thanks!
