My sister sponsored her husband back in april ,2015 and he got his medical done twice ,since 2016 we have not heard from CIC.

It's time for the sponsor to meet the MP. The MP will be able to know what is the reason for not being approved yet. They could also order the GCMS notes via the privacy act, but it takes 30 days to get them. So the fastest would be to ask help from the MP.

Medicals are valid for 12 months, that's probably why they were done twice.

No, not really. Security checks can be long and the delays do not depend only on Canadian authorities. They can be done in collaboration with the CIA, MI6,...
From what I read, they are usually done in 6 months, but sometimes it takes longer. I hope they will get approved soon.

Was the request to redo medicals relatively recent? If yes, that would be a good sign. If they asked to redo the medical, that's probably because they expected to have security check finished soon and they didn't want to add any delays once the security check will be done.

Then I'm afraid there is not much they can do beside waiting. They can continue to contact the MP every month for an update, but there is nothing to do to make it faster.
