Applying study permit for masters inside Canada for sx1 visa holder

Hi there,
Urgent reply needed please. One of my friends who is already in sx1 visa valid for six months got admission in a 2 year masters program. He completed a diploma less than six months as the visa category permitted (SX-1 : Student exempt from the requirement to obtain a study permit) He is inside Canada and wants to apply for study permit. Questions are
1. Which one is the CIC office for application inside Canada
2. Does he need to be outside Canada during application or visa grant day for new port of entry or inside application is ok?
3. What documents and application packages need to follow?
