Applying online for GCMS notes

Guys, so for GCMS i went to the following link. One of the steps is to submit documents. What documents are we required to submit?
Is it the request form that we have to fill online and submit it? I am currently in Canada, so do i also have attach/submit my proof of current residence as well?

If you are in Canada, no documents required. Its optional

Correct, if you apply from within Canada and you request notes for yourself and not for anyone else, then
you do not have to upload additional documents.
