Applying for study permit - need advice

I applied for permanent residence to NZ in 2006, during that time i had given work offer letter which was fake. This i came to know when my NZ's Visa officer told me that i submitted false information during investigation. Later i consulted a lawyer they investigated and gave the details to Visa officer with all the references that i was cheated. I did not get any reply from Visa officer till 2011, so i withdrawn the application some one year back. They permit me to withdraw my application but they did not refund my visa fee.

Now i'm planning to apply for PhD in Canada in Computer science since i completed my master's few months (Aug 2013) back, do i need to reveal the withdrawn application information or not?

There is a question asked in the study visa form "whether you have been denied any visa to any country", though i have withdrawn, should i tell this incident or it is not necessary?

can anyone please advise me?
