applying for PR second time

I became a PR in 2005, did not spend the necessary time in Canada, but when it was time to renew the PR card in 2010 I wrote an application and stated falsely that I had spent the required 2 years. I got a letter approving my application and telling me to pick up my card. By that time I had regretted saying incorrect information and decided that I did not want to put myself in a situation where I would not be able to verify my claims. I never picked up my card and went back to my country.
I am now thinking of applying again for PR and doing everything right this time. My questions are:
1. Is it true that I must first officially relinquish my PR status by applying for a travel document, which will be denied, thereby telling me I am no longer a PR? (Even if I state in my travel document application the same timeframes in my PR renewal application, it will be denied, since the time I claimed I spent in Canada does not add up to 2 years in the last 5 years. I know that and am OK with it).
2. In my new application for PR, when it comes to the part where I say where I lived since I was 18 should I say the truth or take into consideration what I had claimed on my PR renewal form. I would love to just tell the truth, but I am afraid they will compare this to my previous PR card renewal form and find inconsistencies.
3. Will having been a PR before make my chances of being accepted again bad?
4. What if I just want to get a visitor's visa to visit relatives in Canada? I still have to state all the places I lived in. What do I say? The truth or the same as in my regretful PR card renewal application??
Would really appreciate a quick answer from someone like Leon.

I'm sorry I am no expert and cannot give any assistance on this. However, I do have a question.

2. You stated that you regretted lying on the renewal form and that is why you did not follow through with that process, etc .......Yet, in this question, you ask whether or not you should fill out the new application in line with the false information you had given on the renewal form. That seems a bit contradictory to me.

I only said that because I do not want to get into trouble for the incorrect information I put in before. I stated that if I could do everything correctly this time I would be more than happy. My only concern is the inconsistency between the two applcations.
