1) It is mentioned on the website that you must have proof of your child immunization for few diseases. I have a hospital card showing tick marks and date of immunization against those disease. It is okey or do we need a letter from the doctor certifying the same.

2) Also is it compulsory to get enroll for CIIP Program? Please rely

Sorry I can't help you with 1), but 2) CIIP is completely optional - you do not have to attend. However, it is FREE and very useful so it's certainly worth at least considering.
