Dear All

I have received another request of updated docs and a form called "Immigrant Summary Form":

In Instructions it says (It says must be completed in duplicate for all persons over 18)

This form has date for principal applicant (me) spouse and dependent (child)

1] First Question is I have already filled this form with me as a principal applicant, now should i take another copy of this form and fill it again with my spouse being principal applicant ? (data will be identical ) except that the information in the columns will be reversed(me as a spouse).

The subsequent pages of the form have a questionnaire to be completed by all applicants aged 18 or over --


2] I also want to know whether to fill in information for our 2 yr old child in the form or not - - ?

Finally today received call at my home for asking to submit the PPR.

And also received the e-mail stating that your medical is valid until xxxxxxx. Submit your passport for further processing and be prepared to land in Canada before this date.

Now in very short span i have to complete so many things.

But still e-case "In Process" Only.

The letter received from CHC, UK, containing the advise for medical examination & submission of RPRF mentions to use the enclosed reply label to respond to their letter but I haven't found separate reply label enclosed. I have to send RPRF DD but not sure where to send it exactly. I will highly appreciate if forum members who have already send their RPRF DD share their experience and confirm the address where they forward their DD

Please clarify ;

* Does the reply label for submission of RPRF Bank demand draft contain the following address;

High Commission of Canada
Immigration and Medical Service Division
Macdonald House
38 Grosvenor Street
London W1KAA
United Kingdom

This is the address which is mentioned on top left side of letter received from CHC, UK. and also mentioned by one of the member as address on reply label received to him.
