Application to voluntarily renounce PR status - Single before, married now

Hi, I would like to renounce my PR card as I would like to apply for an eTa. I just want to know how exactly do I answer the form. I was single when I was granted a PR status, then I came back to my home country and my PR card expired. Now, I am married and have changed my last name. In the first question, asking my name, it said specifically that I should put the name as it appears on my Record of Landing which would be my maiden name now. However, there is another space below that is asking for Other Names (for example, maiden name, son/daughter of, other family name, other given name). Should I put my married name? Also, in question number 5, it is asking for my marital status. Do I put my marital status when I first got my PR card or my marital status at the present time? The same way in question number 8, asking for my passport number. Does that mean my valid passport now or the passport I used when I first entered Canada, the passport with the Canadian visa. Sorry, these questions might seem stupid but I just really want to avoid the hassle of sending mails back and forth with the immigration. Thank you in advance for your response!
